Application Requirements:
1. Application Letter indicating the position.
2. Fully accomplished Personal Data Sheet (PDS) with Work Experience Sheet and recent passport-sized picture or unfiltered picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph, digitally signed or electornically signed.
3. Hard copy or electronic copy Performance rating in the last period (if applicable);
4.Hard copy or electronic copy of proof of eligibility/rating/license; and
5. Hard copy or electronic copy of Transcript of Records and other pertinent documents to include Relevant Service Record, Certificate of Employment, Certificate of Relevant Trainings and Seminars attended and;
6. Preferably Face-to-face Teaching Demonstration. However for legitimate valid reasons, Demo teaching via zoom may be accommodated.
